Syphilitic mesoaortitis in tertiary syphilis is grossly featured by wrinkling and scarring of the intima (tree-bark appearance). Histologically, destruction of the elastic tissue and smooth muscle layer is evident (HE, low power).   Infiltration of lymphocytes, plasma cells and macrophages is seen around the vasa vasorum in the scarred media (HE, higher power).
